Background Check Instructions
All Leaders, Prospective Members, Employees and Volunteers must have a CLEAR background check. The cost of the background check is non-refundable and the fee is $45.00 per application.
- You are to review and read the consent and disclosure form. If you agree with the form, you will be required to electronically sign the form before it can be submitted for processing.
- The questionnaire must be completed in one session, taking approximately 20-30 minutes. Do not submit your payment unless you have at least 30 minutes available.
- Please be prepared to provide the Region and/or Chapter Name of the region/chapter of interest. You will enter this information on the personal information page.
- Before final submission, please proofread and verify the information you have provided. NO CHANGES CAN BE MADE AFTER SUBMISSION.
- Background checks containing errors will require a resubmission with new payment.
